Here is how to fill out and submit the form you are requested to complete.
- Go to the Forms section of your user dashboard in CaseActive hub.

- Once there, click on “Your Uncompleted Forms”. All the forms that you need to fill out and submit shall appear before you in this section.

- Click on the submit form in the hover-down menu, which is attached to the form under the “Action” column.

- You shall be taken to the body of the form, complete it, and hit “Submit”

Note: You can also find these Forms in “To Do” section in the bottom right of your case active dashboard.
You can also access them through the notifications by clicking the bell in the top right corner and then clicking on the corresponding notification.
